As MDS coordinator, I am responsible for;

• Performing nursing assessments (MDS)that are submitted to CMS for reimbursement , gathering data from multiple avenues such as the medical record, hospital records, staff interviews, patient and patient family interviews
• Assess for subtle changes in patients via the MDS assessment tool and RAI process, monitoring for decline/changes in condition, weight loss, etc
• Participate in gathering and analyzing data and performing audits in regards to quality measures that are direct results of the MDS
• Perform chart and MDS audits to ensure accuracy and organization
• Participate in family care plan meetings and in the development, review, updating and implementation of patient care plans as indicated
• Required to have a strong understanding of the PDPM and ICD 10 coding
• Effectively communicate with members of the interdisciplinary team including doctors, specialists, nurses, and therapists
