Since employed with Kindred I originally was a case manager and managed a patient load up to 21 pts at a time. During this time, due to the autonomy of the hospice field, my assessment skills and background helped me be "the eyes" for the hospice MD's when relaying the patient's needs and issues that would arise in order for them to be properly treated medically.
I was then promoted in 2017 to clinical manager and managed the other 10 nurses as well as training and education. Then in 2019 I was promoted to the Executive Director position
Managed a case load of 12-20 patients in all aspects of their health and psycho social needs. Coordinated care with other disciplines to promote an optimal plan of care to best suit the patient and their families. Provided education to the pt and cg's related to disease processes, medication management to promote quality of life as well as end of life education
Diagnosed and treated high-volume caseload in busy emergency room setting.
