Has an overall responsibility and accountability for coordinating all aspect of the individuals care. Including but not limited to health, and behavior healthcare, community support, and other service required to meet the needs of the individuals. Assist staff and residents with troubleshooting unexpected issues that arise (individuals behavior, medical needs, incidents and staffing issues and ect.)

Assisted with mobility, transferred patients to and from entities. Assisted patient with activities of daily living include eating and drinking, bathing, oral care, grooming and elimination. Changed bed linens as needed, and as part of routine. Regularly checked vitals and administered medication orally. Always ensure a safe and respectful environment.
